No, but they look very similar to the Cameradactyl camera and also some available from Chroma camera which I have seen in person and they're very impressive.
 
I printed two - a 58/65mm body that I use with a pinhole, and a 90mm that has a 90mm f8 Super-Anglon installed. See my Will Travel page for details. The pinhole works much like any other 4x5 pinhole camera.

The 90mm is a bit more interesting, as it does not offer movements - it is essentially a big box camera. You have to rely on depth of field and choice of viewpoint entirely. One useful thing about the design is that it is intended for left and right handed users, with the film holder inserted from one side or the other. This means if you are using a half dark slide you can flip the camera over to get the same amount of rise or fall (or shift if vertical) for a duplicate exposure. You probably want to put together (or find) a suitable viewfinder if you do not use the ground glass.

I have used mine with 4x5 and roll film holders in 6x9 and 6x12. 90mm is wide to wide-ish on these formats. I already had the lens when I decided to make the body, so the expense was basically the reel of filament and the electricity.

The only 4x5 examples from the 90mm I have on the web are part of a developer test at PC-512 Borax test .
 
I have two 3d printed cameras, a Chromer Snap Shot 4x5 and a Noble 6x17, both very good capable cameras.
 
I've got a Chroma Snapshot, works really well with a 65mm F8 and an external viewfinder.
